Introduction
You are invited to take part in a survey to evaluate the supportive care needs of people impacted by cancer. Information from this evaluation will help Cancer Council SA refine its services to better meet the needs of South Australians impacted by cancer.
You are eligible for the study if you are:
- an adult (18+ years)
- living in South Australia
- were diagnosed with cancer in the past 5 years, or are an informal carer (i.e., family or friend) to a person diagnosed with cancer within the past 5 years.
What does participation involve?
Participation involves completing a survey about your supportive needs related to cancer. The survey will take approximately 20-30 minutes to complete. You may consent to participate in the survey once you have read this information sheet and clicked the ‘next’ button. You will then be linked to the online survey.
Depending on your answers, you may be prompted to optionally enter your contact details for a call back from a Cancer Council SA Information and Support nurse. After completing the survey, you will be given the chance to win 1 of 5 $100 gift cards by providing your email. You will also have the option to enter your email to complete a follow-up survey. Entering your email address for the follow-up survey or prize draw will not have any effect on your anonymity as it will be collected separately to your survey answers.
Voluntary Participation and Privacy
Participation in this survey is entirely voluntary. If you do not wish to participate, your access to Cancer Council SA services will not be affected. You will not be identified in the reporting of results as data will be de-identified.

This project has been approved by Cancer Council Victoria’s Human Research Ethics Council (HREC #2232).
